Looking For An Already Existing Add-On For Moderator Notes

I seem to recall coming across a (third party) forum add-on which would allow moderators to make notes about specific members on their profiles (but would only be visible to moderators or members with the necessary permissions).

I cannot seem to find this add-on again.

Does anyone happen to have a URL for it?
