Teleportation to player?

Hello, I am very new to housing and I have a project in mind that includes using a custom item that teleports you to the player that has hit you last. I have the timer and cooldown mechanics working, but I cant seem to find a way to teleport to a player, or even set a teleport destination on the fly which you can use the item to teleport to.
