Bug Report page bugged, can't report a bug

I just encountered a bug of forum that isn't reportable - the bug of Bug Report form itself

I have a bug of Skyblock to report, but when I opened the report page as I always did, no form showed up.
I have tried different browsers, all received the same result.
I discussed this with my guild member, but they don't have any problem like mine.

Please help!
